As our client, you have the right to:

  • Inspect and Copy: Request electronic or paper copies of your PHI. We may charge a reasonable fee for copies.
  • Amend Records: Request corrections to inaccurate or incomplete information in writing.
  • Request Confidential Communications: Ask us to contact you through specific methods or at specific locations.
  • Limit Uses: Request restrictions on how your PHI is used or shared with insurers and family members.
  • Accounting of Disclosures: Obtain one free annual accounting of how your PHI has been shared.
  • Receive Notice Copies: Request paper copies of this privacy notice at any time.
  • Designate Representatives: Authorize someone with medical power of attorney to act on your behalf.
  • File Complaints: Contact us or the U.S. Department of Health and Human Services Office for Civil Rights if you believe your privacy rights have been violated.
  • Opt Out of Fundraising: Request to stop receiving fundraising communications from us.

Our Uses and Disclosures

We use and disclose your PHI for the following routine purposes:

  • Healthcare operations and business administration
  • Billing and payment collection

We may also disclose PHI without your authorization as required or permitted by law, including for public health reporting, law enforcement, legal compliance, workers’ compensation, organ donation, and approved research purposes.
